Care Crisis and No Care Strategy?

On a daily basis, the media reports on the ever-increasing perception of nurses’ deficit. Operations are moved and stations closed. Even the “Day of hand disinfection” on 12 November 2017 was terminated prematurely by many clinics, as a correct hand disinfection cost about 2 hours per employee and shift. Time taht the nursing staff does not have. This already entails up to 60% documentation effort, unstructured processes and the supply of up to 13 patients per nursing staff. And yet, pen and paper are still used for documentation in more than 70% of the facilities [in Germany].

Where is the Care Strategy?

In all the dramatic reports, it should be considered that the relief of nurses in all health care facilities should be at the top of the nursing strategies. But the drama often begins there. Many facilities have a medicine strategy, but not a nursing strategy? As an just-there-anyway resource and cost factor, care was not rated as really system-relevant. Now it is more important than ever that the health care institutions set the long- and medium-term targets, in particular in terms of process control, digitization and qualification. These should then enter into a nursing strategy with appropriate measures, defined resources and a cost-benefit analysis. Such a strategy can already be used to anticipate future decisions (on care offers and concepts), as well as defining a framework or decision-making framework for personnel, budget and investment planning. And the most important thing is that the nurses receive transparency on the planned nursing goals and concepts of the respective employer. This is the only way to clarify the goal of the trip and increase the attractiveness of the work environment, as well as the employee loyalty. Who would like to work with an employer who defines the goal of a satisfactory care? (Note: I have already said in a clinic)

Last but not least, the nursing strategy should also be the prerequisite for all tertiary and, of course, the IT department. This is the only way to check the requirements for a technological support that is easy to maintain against the real implementation (for example in the HIS) and to justify necessary adjustments or procurements. In this way, the frequently non-goal-oriented discussions about “clinical software only from one source” and “we do not want interfaces” can be terminated and finally genuine benefits and relief for nurses are created. And not just the expenses can be reduced, but also the costs and risks. It is astonishing that because of the lack of specialist skills in the nursing care, more and more operations are postponed, old people’s homes and stations have to be closed, and more than 70% of nursing staff still documents with paper and pen. Digitization relieves the burden of care and generates revenues!

As a rule, the nurses themselves do not usually decide about their digital tools. And so it is hardly surprising that the administration or the IT departments often have their own view and arguments on these topics. And these are very often the same:

“We are already doing care in our HIS and want everything from one source” – How lucky is the nursing staff? Has your HIS provider ever shown you how much time and effort the nurses save? How strongly does the HIS systematically automate?
“We do not have time and money for the introduction of a care software” – What does a closed station cost? In addition, a modern nursing expert software even generates revenues and can pay for itself. And in the absence of time one must inevitably think of the wood catcher, who is struggling with a dull ax. He needs three times longer, but has no time to sharpen it.

The establishment of a staff unit “Digital transformation in the care” and the occupation with a care informer is a sensible first step before digitization projects in the care. Trained nursing information technicians (nursing informatics) in Germany will certainly be searched for in vain. Nursing specialists with appropriate qualifications are then more likely to be found.

But what is nursing informatics?

There are many definitions of the term “nursing informatics”. While in the German-speaking world nursing information was defined as the application of computer science in the professional care of people, the term “nursing informatics” exists in the English-speaking world, which goes far beyond this definition.

“[…] nursing informatics more, namely, the combination of computer science, information science and nursing science.” (1989 Graves and Corcoran)

 

This definition shows that nursing informatics is not just the use of information technologies. This also shows the intersection of information science and nursing science.

For the European area nursing informatics can be defined as follows:

 

 

“Nursing informatics is the integration of information and nursing science into health care and healthcare through information and communication technologies. It is a support process of nursing and training, as well as care, project, process and quality management in health care facilities “(H.Mania 2008)

 

What competences should a “nursing informatic” bring or acquire?

Since in Germany no direct course of study “nursing informatics” can be documented at universities, interested nurses have to qualify elsewhere. For this, it is important to know what competences a nursing assistant needs in his professional life. In summary, a “nursing infomatic” must understand the complex relationships between the following elements:

  • Data, information, knowledge and wisdom
  • Nursing science, information science, computer science
  • Nurses, people, health and the environment
  • Information structures, information technologies, information management and communication of information.

In addition, a “nursing informatic” should have knowledge or skills in the following areas:

  • EHealth, mHealth and clinical IT systems or IT infrastructure including interface standards such as HL7, DICOM etc.
  • Usability and software development
  • Regulatory requirements for medical devices, especially software as a medical device
  • Support and training
  • Terminologies and safeguarding the future of care
  • Data protection and data safety
  • Technology lifecycle (planning, process and requirement analysis, specification, selection, training, adaptation, introduction, evaluation, risk management and operation)

Digitization in nursing will be coming and the working environment will change dramatically. The management could prepare for it and thus use the potentials or ignore it and thus be overwhelmed by it.

Innovative technologies for care have been available for a long time, but only about 27% of German nurses are supported digitally.
Many decision-makers, manufacturers and nurses also believe that they already have digital care documentation just because digital forms are available. The nurses have their own and special requirements for digital support. To this end, the experience of care with IT in recent years, characterized by documentation for other professional groups, is rather negative. And since nursing information is not a solid component of education, the IT competencies in care are rather heterogeneous.

So how can nurses be prepared for digital transformation?

The topic of digital transformation and the underlying methods and technologies must become part of nursing competence and nursing care. This can certainly not be done by means of instructions or “presetting” of IT solutions.
An organizational development and management is required. The digital participation of care requires appropriate organizational structures and competences in nursing.

The establishment of a staff unit “digital transformation in the care” and the occupation with a care informer would be a sensible first step. On the one hand, this nursing assistant can represent the interests and requirements of nursing staff in IT projects and projects and train the nursing colleagues. In addition, he advises the management as well as preparing and analyzing the key figures.

On the stations and nursing areas, nursing IT key users should also be named and qulified. These key users are coordinated by the “Digital Transformation in Care” office and integrated into all relevant projects. IT key users in care are also the contact persons for their nursing colleagues. They determine the IT training needs of the students and coach directly in the work environment. They also provide on-site support and pass on the requirements to the staff. They are therefore also the key to a careful and beneficial use of technology.

Such a structure requires corresponding process, project and IT competences of the nursing staff assigned to these tasks. This is the only way to properly assess existing processes and technologies, and to define and implement the requirements correctly.

The Health Care Initiative (IGW) is currently publishing a position paper on care 4.0. The position paper is to be the starting point for an intensive entrepreneurial but also a professional policy debate. The guiding theme of digitization must be taken up and used much more strongly than hitherto. The frequently deficit-oriented view of “care” is presented to innovation, development and competence. For nursing management, new and outstanding fields of action are being opened up through care 4.0. In particular, the topics of process control and patient orientation can be filled with completely new accents.

The present position paper is based on a joint expert workshop of the Initiative Gesundheitswirtschaft in cooperation with the University Hospital Hamburg-Eppendorf and the NursIT Institute GmbH in March 2017 in Hamburg.

Nursing Care is actively involved in digitizing – “Network for Innovation and Technologies in Nursing”

The “Network for Innovations and Technologies in Nursing” (NITiP) will be launched on 31.05.2017 with the virtual nursing conference “Innovations for the nurturing experience …”. The aim is to bring together providers of innovative technological products and services and nurses, and to discuss new solutions on the market, successful projects and future developments.

The working environment of occupational care is changing more and more rapidly, and the complexity, pressure and professionalism are becoming increasingly problematic. Digitization of care is increasingly viewed as a solution approach. More and more often, nurses are being nurtured to actively organize the digitalisation of their profession.

However, in reality, nursing staff and care management often have no idea what solutions and services are already available in the market and which innovations are already “at the door”. In addition, they have too few opportunities to actively influence the development of technologies and services. As a result, many products and services are being developed to meet the needs of nursing staff and the real care process. In addition, there is also a lack of a nursing organization in Germany that is actively working on this topic.

For this reason, NursIT Institute is launching the new “Network for Innovations and Technologies in Nursing” (www.NITiP.de). Through information events and workshops, the technological and innovative possibilities of the market and the real requirements of the users are to ensure synergies on both sides.

The virtual nursing conference “Innovations for the care of life …” (www.PflegeIT.de) will take place on 31st May 2017. The special feature of this conference is that it is conducted exclusively online (in a webinar form). Participants can come and go according to interest, virtually. All lectures are recorded and can also be used later for interested nursing staff. Many new, innovative themes, projects and success stories are presented to the participants. The entire event is free and requires only a short registration, a PC or a smartphone. The organization of journeys to conferences, which is often difficult for nursing staff, is thus dispensed with. At home, at work or mobile – the participation is child-friendly.

In the future, care-technology topics will also be worked on in the network, positions will be developed and joint projects promoted.

The conhIT 2017 was again a great success – even bigger and even more exhibitors. The largest German Health-IT conference and fair attracted interested and decision makers in the health sector. Care is also gradually becoming the focus of this event. So there were some lectures, panel discussions and an industry management on the topic of “care”.

We from the NursIT Institute again were present at the Berlin-Brandenburg Community Stand, as we presented the latest developments of our nursing expert software CareIT Pro and our service around the subject of “Lean Nursing” with a strengthened team.

What could we take with us in terms of care from conhIT 2017 in a nursing perspective?

Many actors in theory deal, in the form of current studies, with the digitization of care. Thus, among other things, The panel discussion “Digitization in the care – When does the knot burst” took place. It was also striking that when nursing was the topic, usually the outpatient or long-term care is meant. Why is clinical care so little on the radar of the actors?
Of course, there were also some manufacturers of nursing software present.

All in all, it is certain that there is still much to do in order to anchor the modern technologies in the care sector. This includes the fact that the care itself should organize and engage itself in order to determine the requirements of the own digital tools themselves. But also the creation of incentives for the introduction and use of digital care documentation was mentioned. Another point is the lack of advice and training in the area of ​​nursing technologies.

Yes! The organization of care in a network or association for nursing ICT is necessary. 10 years ago, I was the chairman of the “Deutsche Gesellschaft für Pflegeinformatik”. This association of 40 nurses was apparently ahead of its time and has disappeared after 3 years. Today, exactly the same company is needed to help shape technological developments. Therefore, we are currently working on reviving the idea and starting the “Network for Innovation and Technology in Care”. More information soon 🙂
